Groundwork Health, PLLC ("Groundwork Health," "we," "us," or "our") operates groundwork-health.com (the "Website"). This policy explains what information we collect through the Website, how we use it, and the choices you have.

This policy covers the Website only. If you become a patient, your health information is protected under HIPAA and handled as described in our Notice of Privacy Practices, which you receive at enrollment. Information you share on the Website, including our email list, is not part of your medical record and is not protected health information.

  • Information you enter into contact or inquiry forms, such as your name, email, phone number, and message

If you join our email list, we'll send you emails about Groundwork Health. You can unsubscribe at any time using the link at the bottom of any email. Signing up for our email list does not create a patient relationship.

How long we keep information

We keep email list information until you unsubscribe or ask us to delete it. We keep other website information only as long as needed for the purposes described in this policy or as required by law.

Age requirement
