Direct primary care is a membership model that removes insurance from the equation entirely. You pay a flat monthly fee and receive care that is actually coordinated around your life, with a provider who has time to show up for you every single time.

No copays, no referral hoops, no surprise bills
For many patients with high-deductible plans, membership costs less than what they already pay out of pocket. For everyone else, it is simply a better way to access care.
Groundwork Health DPC launches August 1, 2026 via telehealth, serving patients anywhere in Tennessee. In-person visits in the greater Chattanooga area begin in early 2027.

LABS
Groundwork Health maintains a wholesale lab agreement that allows us to offer significantly reduced pricing compared to standard retail rates. To take advantage of this pricing, labs are billed through the practice rather than directly to you. Payment is due prior to order submission. You will be invoiced at our exact cost with no markup. Labs are not billed through insurance.

WHO IS DPC RIGHT FOR?

Groundwork Health is an adult-only practice serving patients 18 and older.
DPC does not replace emergency care or specialist services. Members are encouraged to maintain a catastrophic or wraparound insurance plan for hospitalizations and specialist referrals.
TELEHEALTH AND IN-PERSON
Groundwork Health DPC launches August 1, 2026 as a telehealth-first practice, serving patients anywhere in Tennessee via HIPAA-compliant video visits. In-person visits in the greater Chattanooga area begin in early 2027.
All existing members keep their rates when in-person visits launch. No price increases at any point in the transition.
